  I cannot see more than three questions

  If you are asking for help, provide lots of detail so problems can be reproduced.
  
  Things to describe:
  Operating system ATutor is installed on - Linux
  ATutor version - 2.2
  Patch #s applied - none
  ATutor theme name - Atutor 2.1
  PHP version - 5.3
  MySQL version - 5.5
  Webserver & version - Apache latest
  Copies of error messages -
  Changes to default settings - none
  Web browser being used - Firefox 36
  ...and anything else relevant -
  
  Hello I downloaded Atutor 2.2 from Sourceforge.
  
  I successfully set it up on my shared hosting at bluehost.com
  
  I can create more than three questions but the question bank only displays three.
  
  Therefore when trying to create an exam I can only select those three and no others.
  
  I know this must be something I am missing

  As far as I can tell the question bank is working okay.
  
  Look in the database table tests_questions and see how many exist there.
  
  If you are pasting from a Word doc, MS quotation marks will break data. In such cases delete the MS quote after pasting, then retype it manually.
  
  Also make sure you haven't selected a category of questions, as opposed to All Categories.
  
  The question bank is specific to a course, so you won't see questions created in other courses.

  So are you saying that when creating a question i should not put it into a category on the question creation screen. Or that I should not select the category before creating the question.

  Use categories, but just be sure that you're not just seeing three questions because you've selected a category with just three questions.
  
  Sounds to me, based on your response, that categories is not the problem. Check the other potential issues.

  Okay I should clarify a little
  
  There is only one course and only one category of questions.
  
  The system seems to let me create more than three questions but no more than three are displayed in the question bank regardless of what category i select.
  
  I get no errors when creating the additional questions.
  
  The table tests_questions shows only the first three questions i created.
  
  Also I am only creating questions and answers from plain text.
