  Where can I create content and how can I associate the content to a course?

  Click on the Manage tab when logged into a course as an instructor, then in the content menu click "create." There's also the editor toolbar that appears near the upper right, and the content navigation editor bar that can also be used to create content.
  
  You do not need to associate content with a course. That happens automatically.
